How Do I Read These?

Our forms are in PDF format. PDF stands for Portable Document Format, and PDFs are readable on any computer that has a PDF reader. If you do not have a PDF reader, you can obtain one, free of charge, from Adobe.

After installing the Reader, you can download the form, save it, and open it with the Adobe Reader. If it is a fillable form, you can fill in the fields in Adobe Reader, and then print out the form. (You will not be able to save your changes, however).
